The Per Diem Social Work Counselor plays a critical role in supporting this mission through direct clinical service.
Within the Social Work department, the Per Diem Social Work Counselor provides essential psychosocial services to patients, families, and caregivers across the cancer care continuum. The Per Diem Social Work Counselor collaborates closely with a multidisciplinary healthcare team to reduce psychosocial barriers to care and to support treatment adherence, advance care planning, and overall patient well-being. The Per Diem Social Work Counselor contributes to patient care in an onsite clinical setting at the Texas Medical Center.
The ideal candidate is a licensed social work professional with a Master's Degree in Social Work from a CSWE-accredited program and current licensure as an LMSW or LCSW in the state of Texas, with the ability to deliver biopsychosocial assessments, counseling, and care coordination in a complex healthcare environment.
The hourly rate for this position is a flat rate of $52.00 per hour. The typical work schedule is two weekdays per week, 8:00am-5:00pm, with no weekends and no more than 19 hours per week. The work location is onsite at the Texas Medical Center in Houston, Texas.

Responsibilities
• Conduct comprehensive biopsychosocial assessments using appropriate diagnostic criteria to develop and implement individualized treatment plans
• Provide counseling services to individuals, families, and groups across the disease trajectory, including end of life, survivorship, and bereavement
• Support patient compliance with treatment protocols through education, counseling, and coordination of care
• Facilitate advance care planning discussions and documentation
• Participate in continuity of care and discharge planning activities
• Link patients and caregivers to internal and external resources to address psychosocial needs
• Document all patient care activities in the institutional electronic healthcare record in accordance with departmental documentation policy
• Collaborate with multidisciplinary healthcare team members to support holistic patient care
• Perform other duties as assigned
