Care Manager

The Care Manager at MD Anderson Cancer Center provides transitions of care through phone and online communication for patients, families, and caregivers following their visit to MD Anderson. This role serves as a frontline clinical resource, helping patients navigate post-discharge needs, close gaps in care, and access appropriate resources while improving patient outcomes, reducing unnecessary readmissions, and enhancing the overall patient experience.

The ideal candidate will have a Bachelor of Science in Nursing (BSN) from an accredited school of nursing, three years of oncology experience in a hospital setting, and an active Texas RN License. Experience in a telephone help line, counseling, teaching, customer service, or population health setting is preferred, and bilingual Spanish/English skills are highly desirable. The ideal candidate must be available to work varying schedules that support coverage during business hours and after-hours Monday through Friday, as well as 24/7 coverage on weekends and holidays.

The candidate must be available to work four 10-hour shifts per week on a remote basis, unless ride-out is activated.
Weekdays: 2:30pm - 1:00am (Monday - Friday)
Weekend Rotation: 10:30am -9:00pm - Every other weekend (Saturday-Sunday)
Holiday Commitment: 1 major bundled holiday per alternating rotation (see example below) AND 2 minor holidays. For example:
Year 1: December 24, 25, & 26 + minor holidays (2)
Year 2: Thanksgiving Day, the day after, & New Year's Day + minor holidays (2); Spring Break
Ride-Out: Essential ride-out commitment when assigned requires on-site presence

askMDAnderson Clinical Programs provides post-visit support and care coordination for patients, families, and caregivers. The team serves as a critical point of contact for clinical questions, care instructions, medication concerns, follow-up coordination, and access to resources, ensuring continuity of care and exceptional patient support beyond the hospital setting.

Hourly breakdown of salary range:
Minimum $48.55 | Midpoint $60.81 | Maximum $73.07

Essential Job Functions:
• Respond to clinical alerts and patient inquiries via telephone and online communication channels
• Address medication questions, care instructions, follow-up appointments, home health needs, durable medical equipment requests, and other post-discharge resource needs
• Close care gaps using approved resources and oncology clinical expertise
• Conduct proactive assessments to prevent complications and improve patient outcomes
• Assist patients and providers with understanding and navigating the MD Anderson referral process
• Utilize motivational interviewing, active listening, and communication skills to support patient understanding and engagement
• Provide timely and accurate follow-up through phone and email communication while meeting departmental standards
• Handle difficult or complex customer interactions using effective de-escalation and customer service techniques
• Conduct service recovery activities and collaborate with Patient Advocacy and Health Information Services as needed
• Accurately document all interactions and patient care activities in the electronic medical record
• Maintain current knowledge of oncology care, institutional resources, and departmental procedures
• Successfully obtain ONCC certification and case management certification within one year of employment
• Participate in continuing education and quality improvement initiatives
• Assist with staff education, onboarding, and training activities as appropriate
• Use performance metrics and departmental standards to evaluate and improve service delivery
• Participate in team projects, departmental meetings, and Team Anderson activities
• Strengthen interdisciplinary relationships and partnerships to improve patient outcomes
• Other duties as assigned

Education Required: Bachelor of Science in Nursing (BSN) from an accredited school of professional nursing
Education Preferred: Oncology Certified Nurse (OCN) by Oncology Nursing Certification Corporation (ONCC) or equivalent; must obtain within one year of employment if not already certified
Experience Required: Three years of oncology experience in a hospital setting; experience speaking with patients and families by phone; experience with computers, data entry, research, and written reports; proficiency in Microsoft Word and internet research
Experience Preferred: Telephone help line, counseling, teaching, customer service, oncology navigation, or population health experience; bilingual Spanish/English
Licensure/Certification Required: Current State of Texas Professional Nursing License (RN)
Licensure/Certification Preferred: Oncology Certified Nurse (OCN); Case Management Certification
