Pharmacist Inpatient - Dayshift
The Pharmacy Division is dedicated to ensuring safe, effective, and cost-conscious medication use while collaborating with healthcare professionals to achieve optimal drug therapy outcomes. The Ideal candidate is a licensed pharmacist with a Pharm.D. or Bachelor's degree in Pharmacy from an accredited college. They demonstrate strong clinical knowledge and a commitment to patient-centered care in an ambulatory oncology setting. While hospital pharmacy experience (6 months or more) is preferred, the candidate should excel in accuracy, communication, and collaboration within a multidisciplinary team.
Working Hours: The working hours are 7:00a - 3:30p. Mon - Fri

Core Pharmaceutical Services
Core pharmaceutical services include, but are not limited to, the following:
- Reviews, clarifies, and processes medication orders accurately and in a timely manner.
- Ensures appropriateness of ordered medications by considering relevant factors such as:
- Disease state
- Body surface area (BSA)
- Age, height, and weight
- Laboratory values
- Renal function
- Utilizes computer resources and clinical information systems to support pharmaceutical care, including:
- Medication order entry
- Order verification
- Medication distribution
- Medication checking
- Medication reconciliation
- Performs and documents therapeutic interventions as appropriate.
- Actively participates in event reporting using UHC Safety Intelligence.
Pharmacy Area Supervision / Responsibilities / Administrative / Systems-Based Practice
Responsibilities include, but are not limited to, the following:
- Provides supervision of immediate technical staff, including:
- Assigning and overseeing daily tasks
- Monitoring lunch breaks
- Ensuring adherence to institutional and departmental policies and procedures (e.g., sterile and non-sterile compounding)
- Ensures all tasks and assignments are completed in a timely manner to maintain optimal workflow and patient care.
- Maintains a clean and organized work area at all times.
- Monitors inventory and supplies; notifies appropriate personnel of shortages or outages.
- Oversees the compounding of sterile and non-sterile admixtures.
- Completes all assigned Quality Assurance and productivity reports, including documentation and appropriate follow-up.
- Researches and monitors problems, initiates corrective actions, and follows through until resolution.
- Consistently provides input and suggestions for process improvement.
- Completes and maintains all institutional and divisional requirements for licensure, mandatory education, and testing, including but not limited to:
- EEE
- Infection Control
- Age-related competencies
- Code Blue
- BLS certification
- TB testing
- IV process validation
- Develops, implements, and/or supports special projects throughout the institution.
Clinical / Patient Care
Clinical and patient care responsibilities include, but are not limited to:
- Actively participates in the implementation and practice of identified clinical initiatives (e.g., discharge counseling, medication reconciliation, renal dose adjustments).
- Conducts and documents patient education as appropriate.
- Documents patient-specific information in the electronic medical record.
Other Duties

Education
- Required: Bachelor's Degree Pharmacy. or
- Required: Pharm.D. degree from an accredited college of pharmacy.
Work Experience
- Preferred: Six months experience in a hospital pharmacy.
Licenses and Certification
- Required: RPH - Registered Pharmacist Texas State Board of Pharmacy. within 90 Days and
- Required: BLS - Basic Life Support American Heart Association (AHA). within 30 Days and
- Required: ACLS - Adv Cardiac Life Support American Heart Association (AHA). If working in one of the following departments: Pharmacy Inpatient (710348), Pharmacy OP ACB (710355), Pharmacy-West Houston (710657), Pharmacy-The Woodlands (710480), Pharmacy-NWH Sur&SP Care (711204). within 90 Days
